  • One-Year commitment

  • Early cancellation price of monthly fee of their package and 30-day notice

  • Memberships are non-transferable and non-refundable.

  • Monthly services must be used within the calendar month. Unused services or benefits do not roll over.

  • Discounts cannot be combined with other memberships or packages. Membership pricing and benefits are non-negotiable.

    • Membership fees are billed automatically each month to the card on file

    • Missed or late-cancelled appointments (less than 48-hour notice) will result in forfeiture of that month’s visit

    • One pause allowed per year for up to 30 days with written notice

    • Membership includes services outlined above. Additional treatments, specialty labs, and advanced services are billed separately

    • Membership does not guarantee specific outcomes and is not a substitute for emergency medical care
